Our school is participating in the National Youth Tobacco Survey (NYTS), sponsored by the FDA.
Students in grades 6–12 may be asked to complete a 20-minute online survey
The survey asks about tobacco and nicotine attitudes and behaviors
No names are collected and student privacy is protected
Participation is voluntary
Students may skip questions or stop at any time
There is no penalty if your child does not participate
Survey results help improve student health programs in the future.
➡️ Only return the form if you do NOT want your child to participate (due within 3 days).
A copy of the survey is available in the school office.
